Outpatient Requirements effective May 1, 2007

A complete order is required from your physician to ensure proper care. In order for an order to be complete, the following information must appear on your physician's order:

  • Patient name
  • Identity and signature of the ordering physician, practitioner or designee
  • Services to be performed
  • Diagnoses which supports all tests or services to be performed
  • Date and time

Medicare Patients Only

Medicare rules indicate what tests and treatments are covered given the diagnosis from your physician. You may be financially responsible for outpatient testing services which are not covered by Medicare. We will tell you which tests are not covered prior to having them performed.
