Jack S. Bartoszek Relocates to Edwardsburg Family Medicine

EDWARDSBURG, MI – On October 8, 2009, Jack S. Bartoszek, D.O., will be moving to Edwardsburg Family Medicine, an affiliate of Elkhart General Hospital.

For patients wanting to continue with Dr. Bartoszek, they can contact Family Medicine Center at (574) 875-5126 and sign the Authorization to Release Medical Records Form to transfer their records to his new location. Patients who prefer to remain at Family Medicine Center can make an appointment with either Cynthia Bontrager, M.D. or Melissa Zimmer, M.D., so they may follow with their current medical care.

Dr. Bartoszek is board certified in Family Practice and Geriatrics. He graduated from Chicago College of Osteopathic Medicine, Chicago, IL and completed his residency from Michiana Community Hospital in South Bend, IN. He is a member of the American Osteopathic Association, American College of Osteopathic Family Physicians, and the Indiana Osteopathic Association.
