Home Infusion Therapy

Home Infusion Therapy allows patients to continue with their daily activities while receiving care in the comfort of their homes and offers a costs effective alternative to lengthy hospitalization or repeat trips to the hospital. Elkhart General Home Care’s infusion team works with the patient’s physician who directs the overall management of the patient’s care.

We focus on managing each component of the therapy through our comprehensive staff of clinicians and case managers, who proactively monitor patient responses and outcomes to prescribe therapies. Our commitment is to provide the highest quality of care, clinical excellence, and customer services.

Your Health Care Team

Elkhart General Home Care employs registered nurses with extensive acute care backgrounds. From the first visit to routine follow-up visits, nurses educate the patient and family members with respect to medication and self-administration techniques. Elkhart General Home Care’s PICC certified nurse clinicians are experienced in infusion management and maintain and problem-solve the patient’s IV site and catheter. Monitoring the patient on an on-going basis includes assessing response services the patient may require.

Our pharmacy staff has extensive admixture experience and is available as a resource for drug information. They are active participants in the patient’s ongoing Plan of Care. Our on-site pharmacy has a complete formulary of drugs and solutions. Infusion pumps and equipment are selected based on the latest technology and most appropriate selection for each patient. Medications and supplies are delivered to the patient’s home in limited quantities to control drug costs due to therapy changes.

Continuum of Care

Our program provides complete coordination of a patient’s care from the hospital to the home. We can also coordinate the patient’s infusion needs directly from the physician’s office. Often times, we can avoid the patient’s hospitalization completely and begin their treatment directly in the home.

Our experienced team of IV therapy nurses and pharmacists are available 24-hours-a-day, seven days a week to meet every requirement of the treatment plan. We will customize a Plan of Care to the physician’s specifications and the patient’s individual needs. We will assess, teach, administer, communicate and coordinate with all disciplines, as well as provide on-call services, laboratory testing and monitoring and supply the patient with the necessary home medical equipment that will need the infusion needs.
